All lay employees and volunteers should make sure that they acquire their Live Scan Request forms directly from their school or parish offices in order to reduce the number of rejected requests.
If your children attend a Catholic (and you plan to volunteer or work at the school), be advised that you should get the Live Scan request from your SCHOOL, and NOT from your PARISH. That way, you can volunteer in both the school and the parish without paying for a second Live Scan.
It is critical that the school or parish office has filled in all the fields pertaining to the parish or school, leaving blank only the personal information of the Live Scan applicant.
It is also important that the applicant fill out the Live Scan request while present in the office. The office should then make two more photocopies of the request and give the applicant the three forms.
The applicant will take ALL THREE forms to the Live Scan Appointment. Make sure that the Live Scan employee signs, dates, and enters the facility information on ALL THREE requests. They will keep one copy, but the applicant must bring one copy back to the school or parish office, and keep the last one as proof of compliance.
