• Associated with a Parish:  ORI A6109  with Mail Code: 07398

The Parish where a Brother or Sister is assigned will provide the request forms. You will use ORI: A6109 (parish) with Mail Code: 07398. Use that parish's entity number on the request form.


• Associated with a School:  ORI A3004 with Mail Code: 01173

The School where a Brother or Sister is assigned will provide the request forms. You will use ORI: A3004 (school) with Mail Code: 01173. Use that School's entity number on the request form.


• All other Religious Men and Women:  ORI and Mail Code: will be determined
  

The Office of Consecrated Life at the Diocesan Pastoral Center will provide you the request forms, ORI #, mail code and the entity you will use.
